Registration Order Form for Merrie Musicks

I am registering version _____ of Merrie Musicks.

I prefer  ___ 3.5"  ___ 5.25"  format for any disks you send me.

I have noticed these bugs in Merrie Musicks:

1. ____ SEND ME EVERYTHING!  I enclose $22 for the instructions to ALL
      the undocumented features (octave, tempo and style setting;
      displaying words to songs; playing single songs from command
      line; turning off "shave-and-a-haircut"; and playing continuous
      random songs), plus the QuickBASIC 4.5 source code to the
      current version, and send me the NEWEST version now or (if I
      have the current one) as soon as it becomes available.

2. ____ Send me all the undocumented features' instructions, but not
      the source code or the newest version.  I enclose $10.

3. ____ Send me the QuickBASIC 4.5 source code for Merrie Musicks.  I
      enclose $10.

4. ____ Send me the newest version of Merrie Musicks (I have checked
      my diskette format above).  I enclose $5.

5. ____ Send me the instructions for the undocumented features and the
      source code, but not the newest version.  I enclose $18.

6. ____ Send me the instructions for the undocumented features and the
      newest version, but not the source code.  I enclose $14.

7. ____ Send me instructions for setting the octave, tempo and style. 
      I enclose $5.

8. ____ Send me the instructions for playing one song from the command
      line.  I enclose $3.

9. ____ Send me the instructions for displaying the words to sixteen
      of the songs while playing them.  I enclose $2.

10. ____ Send me the instructions for not playing
      "shave-and-a-haircut" on exit.  I enclose $1.

11. ____ I wish to sell copies of Merrie Musicks.  Send me the newest
      version and written permission to sell copies of it.  I enclose
      $25.
