August 22, 1993 Request for Membership: International Association of Modem Users This is a "Request for Membership" into the International Association of Modem Users (IAMU). After completing this Request, please keep a copy of it for your own records. ALL INFORMATION PROVIDED HEREIN BY YOU, WILL NOT BE RELEASED WITHOUT YOUR EXPRESS CONSENT. This Request is nine pages long and contains about 40 items for your consideration. If you print out a copy now, it will be easier for you to fill in your answers. It should take 15 or 20 minutes to complete. This is a serious undertaking, so please read carefully. Required answers are marked with a Plus "+" sign. "Steering Committee" Members are required to donate a minimum of eight hours per month during the term of their Membership, in addition to any special meetings or tele-conferences, as may be required. After we reach a sustainable Membership level, the Steering Committee will be directed to implement a program to assist Members with expenses incurred on behalf of Association activities, on a go-forth basis.(ie. telephone & E-Mail expenses). 2. "Volunteers" are expected to donate at least four hours of time per month, during the term of their "free" Membership. 3. "Supporters" may pay by personal check drawn on a U.S. bank or by using an internationally accepted money order payable in U.S. Dollars. We are in the process of applying for a "charitable/non-profit" status with the state of Washington and will subsequently file for "tax-exempt" status with the Internal Revenue Service. Until these filings are complete, monies received MAY NOT be deductible on your state or federal income tax returns. *Please consult with your accountant or tax attorney regarding any such tax deductions. 4. All Members in good standing are required to vote on Major Decisions that are brought forth by the Steering Committee or by the Founder. 5. As Requests for Memberships are received, they are dated and numbered in the order received. This Information upon receipt by the IAMU, is expressly considered to be subjectively private, personal and confidential. It will not be released to anyone, without your consent, under any circumstances. 6. As with any association, these items are subject to change. Periodic news bulletins, monthly reports, quarterly state- ments and an annual report of the Association's activities will be transmitted to all Members, in good standing. 7. Initial Requests for Membership will only be accepted by Electronic Mail or by way of a Text File Transfer (this proves that you are a Modem User!). As these Requests are received by the Association, they are deleted from the Mail and File Storage Areas under our control on the computer or on-line services that we use to receive them. (See #5) NOTE: The systems that are used to store, transmit or relay your Requests to us, may not be able to guarantee "privacy" or non-disclosure. Interested Non-Member Modem Users may submit information,rumors and opinions to the Association. These will be given to the Steering Committee for verification and action. If you have any questions, contact us by phone or by E-Mail before you send it in to us for consideration. 3. If the Request is found to be inaccurate or incomplete (either before or after Membership is granted), we may ask you to clarify information provided or we may ask you to re-submit a new Request. 4. If you are have knowingly participated in the distribution of those infamous "Modem Tax" hoaxes, you must include an open letter of apology addressed to "ALL MODEM USERS", along with this Request for Membership. 5. You may also ask us to pick up this Request, directly from either your computer system or from your local Bulletin Board System. To pick it up from your computer, we'll have to arrange a specific date and time to arrange for the file transfer. To arrange for us to pick the Request up from your local Bulletin Board System, ask your BBS Sysop to help you with this. He or she will have to notify us when the file is ready for pick up. We would prefer to have this file be prepared as a "password-protected" text file to prevent someone else from downloading it by mistake. (Ask your local Sysop to help you!) 7. After your Membership has been approved we'll send you information that will include details about your Membership and the Association's current activities. The IAMU reserves the right to delay, deny, suspend, modify or otherwise cancel or revoke any or all Memberships and/or remove access to any or all Forums and Conferences that exist for the express use of IAMU Members, as may become necessary to protect the interests, reputations, or safety of the IAMU, or any of it's Members, or in due consideration of same, on behalf of any other associated entities (ie.individual Bulletin Board Systems, Users or Sysops, Network E-Mail providers, or on-line service providers such as Delphi, et. al.).
