METHODS: Peer recruitment in the Central Area of Sao Paulo City has been used to build up an open cohort with transvestites and other homosexual male street sex workers on a weekly contact basis. Safer sex practices are discussed and condoms are distributed at each interview. An initial standard questionnaire was applied and a follow-up control is established. HIV tests are performed every year and syphilis tests every three months. RESULTS: From december/91 to december/93, 243 transvestites and 56 other male sex workers have been followed. 116(49.6%) are HIV positive among transvestites and 17 (30.4%) among the other group. (chi 2 = 6.72; p = 0.01). 118 (50.4%) of the transvestites are positive to syphilis while 12(21.4%) of the other group are in that condition. (chi 2 = 15.36; p = 0.000). DISCUSSION AND CONCLUSIONS: An important statistically significant difference was found. Further multivariate analysis are being conducted to detail the difference between the two groups of sex workers.
