Unique Identifier : AIDSLINE ICA10/94369943 AB OBJECTIVE: To compare the results of pap smears versus colposcopy as a screening test for cervical intraepithelial neoplasia (CIN) in HIV seropositive women. METHODS: Sample 100 HIV positive and 50 HIV negative women, pregnant women excluded. Between 4/93 and 2/94, 76 HIV positive and 20 HIV negative women enrolled. Exam includes consent, complete history and physical exam. CD4, pelvic exam with pap smear and colposcopy, screening tests for STD's (HIV, HPV, RPR, chlamydia, candida, bacteria and trichomonas), and directed cervical biopsies as indicated, are performed at enrollment, 6 months and 12 months. At study completion, all HIV positive women will be offered enrollment in a long-term natural history study of women with HIV disease. FINDINGS: 40% of HIV positive women have abnormal pap smears at entry compared to 10% of controls; 37% of HIV positive women had abnormal Colposcopic exams compared to 10% of controls. 59% of Negative pap smears in HIV positive women are positive for oncogenic types of HPV. 60% of women with negative pap smears are positive for HPV. CONCLUSIONS: There is good correlation between the results of pap smear as compared to colposcopy in screening for CIN in HIV positive women. At study completion, data will be available in regards to the recommended frequency of cervical screening on HIV positive women (6 versus 12 months).
