As patient life expectancy increases due to early intervention with multiple drug therapies for HIV infection, there has been a significant increase in the occurrence of NHL and ONHL. Recognition is difficult since ONHL can appear similar to mycotic infections, dental infections and salivary gland tumors. ONHL is very aggressive, therefore, early diagnosis and treatment is important. Our cases illustrate the clinical presentation and treatment approach in seven patients with ONHL. Patient characteristics: age at onset (35-55); the maxilla was the most common site (6 out of 7); all patients experienced severe pain; all patients had received therapy with AZT from 9 to 60 months; all patients had oral candidiasis. Aggressive multiple cytotoxic drug therapy was the standard form of treatment. Common side effects were fatigue, nausea, weight loss, weakness and neutropenia. Radiation therapy for ONHL was added in 2 patients. Survival times ranged from 5 to 18 months. This is the largest reported series of ONHL.
