Unique Identifier : AIDSLINE ICA10/94369559 AB OBJECTIVES: Hyaluronan (hyaluronic acid, HA) is synthesized by mesenchymal cells, activated by growth factors and cytokines (EGF, PDGF, TGF beta, IGF-1, IL1). Kaposi's sarcoma (KS) is a tumor of mesenchymal origin, and data suggest that the induction of the lesions involves a pathway of events mediated by cytokines. We tested the hypothesis that HA synthesis might be activated and serum HA levels increased in HIV patients, particulary in those with associated KS. METHODS: We measured serum HA levels in normal subjects (n = 40), in asymptomatic (n = 61), in symptomatic HIV patients (n = 12), in patients with AIDS related KS (n = 11). In two patients with KS receiving a chemotherapy, serum HA was determined monthly for 6 months. RESULTS: We found no significant difference in serum HA in asymptomatic HIV patients (39 +/- 26 micrograms/l) when compared with normal subjects (32 +/- 29 micrograms/l). Serum HA was significantly increased in patients with AIDS related KS (317 +/- 299 micrograms/l) and in symptomatic HIV patients (215 +/- 127 micrograms/l), p < 0,01. In one patient with cutaneous KS which received chemotherapy serum HA decreased from 542 micrograms/l before treatment to 181 and 215 micrograms/l, (at 2 and 4 months) when there was clinical improvement. Later, as the KS progressed, HA increased to 658 micrograms/l at 6 months. In the other patient with visceral KS serum HA increased (137 to > 1000 micrograms/l) as the tumour progressed according to the CT scan. CONCLUSIONS: Serum HA levels are increased in symptomatic HIV patients, particularly in those with associated KS. HA might be a useful tool to monitor the treatment of KS.
