Unique Identifier : AIDSLINE ICA10/94369468 AB OBJECTIVES: To identify the major clinical problems and their causes which lead to AIDS and death. To establish the priorties for clinical research. METHODS: Clinical prospective study of adult AIDS patients (WHO definition + ELISA) admitted to 2 hospitals of Kinshasa from january to june 1993. Recording of live-threatening problems and their causes afforded by clinicians. Epidemiological, clinical and biological study. RESULTS: 160 AIDS patients (pts), 67 men and 93 women were enrolled. Socioeconomic conditions: poor (pc) 40%; middle (mc) 35%; high (hc) 25%. WHO clinical stages: Stage III: pc pts 30; mc pts 25; hc pts 25. Stage IV: pc pts 34; mc pts 31; hc pts 15. Clinical conditions: wasting 100%, fever 73%, asthenia/anemia 84%, skin diseases 79%, diarrhoea 58%, tuberculosis 37%, non specific pneumopathy 30%, neurologic diseases 27%. Mortality 45%. CONCLUSIONS: Socioeconomic conditions are cofactors of disease progression. Wasting, fever, diarrhea, tuberculosis, non specific pneumopathy and neurologic diseases are major clinical problems on which clinical research must be focused.
