STUDENT DATA FORM. The primary objective of The Scholarship Search is to help you learn about the scholarships for which you may qualify. Some scholarships are awarded by big foundations, based on your grade- point average and how many activities you participate in at your school. Other scholarships are available only to the children of employees of an individual company. Still others go only to students who are members of a certain church, or whose grand- parent or parent was born into a certain race (Armenian, Indian, etc.), or whose father or mother belongs to a specific club, or was (or is) in the military. Some scholarships are awarded because the student is a member of a club (Boy Scouts, 4H, etc.) or because the student has won a contest (photography, talent show, etc.). The Student Data Form asks you to tell about yourself in these areas so that The Scolarship Search will be complete. - Be sure to answer everything on Page 1 - Fill as many lines as you can on Pages 2 and 3 - Make an "X" for everything which applies to you on Page 4 - Fill as many lines as you can on Page 5 - Write any additional information about YOU on extra pieces of paper - Review all of your answers with a parent to be sure you haven't forgotten anything COLLEGE FINANCIAL PLANNING QUESTIONNAIRE The purpose of the College Financial Planning Questionnaire is to help us make your Financial Planning Report as accurate as possible. Consequently, there are several questions about your parents 1987 Federal Income Tax forms. You should ask a parent to help you with Sections H, I and J of the Questionnaire. Your parent may also have to help with Sections D, E, F and G. So, think carefully about all of your unique talents and background while you are answering the College Financial Planning Questionnaire. - Answer every question in Section A - Answer every question in Section B with Yes or No--NO BLANKS ALLOWED, except for questions 24B and 24C!! - Think carefully about Section C. You probably DO need to answer it - Work carefully with Sections D, E, F and G. They really do make a difference - Remember to get a parent's help with Sections H, I and J - Read the Student Background Questionnaire carefully before you answer it.
