	- mail the order form, with payment, to the address at the
	  bottom of this form
	- registration implies acceptance of the disclaimer of warranty

	__ $15 U.S., Postal delivery of 3.5" diskette, inside U.S. only
	__ $20 U.S., Postal delivery of 3.5" diskette, Canada & Mexico
	__ $25 U.S., Postal delivery of 3.5" diskette, all other nations

(A brief word about delivery methods...  Please accept delivery by e-mail
 if possible.  Not only is it faster, less labor intensive, less costly,
 etc., but we/you don't have to worry about sending a diskette around the
 planet, write/read head alignments, heat/impact/magnetic problems, etc.)
