be printed from Windows Paintbrush
or any other paint program. Black
& white and 16 color format.

If you use Windows Paintbrush, you
must reset your "Page Setup" margins
to keep all the form on a single page.  
Set Top, Bottom, Right, and Left 
margins to "0" before printing.

Have fun... change it as you like
