     I have filled out every blank on this form and request that you include
     my BBS in your next list.  Thank you!  I understand there is NO CHARGE
     for inclusion to the Tri-State BBS List and that my ad will continue
     to be printed in every issue until I ask you to omit it.

     I understand Tri-State BBS List will be released on the 6th day of every
     month and I will let you know if I've made any changes and would like
     those changes to be included in your next issue.  I will contact you
     if any information in the listing conserning my BBS is incorrect or
     if I would like you to omit my advertisement.  I fully understand that
     inclusion in the Tri-State BBS List is entirely voluntary and that
     neither Night Shift BBS (Official Distribution Site), or Leland Mohawk,
     is under any obligation to include my BBS in your listing.
