  This file contains the registration instructions and then an order form. You
should print out the whole file and keep this first page for your records. The
second page is the order form.

  For a personal registration for Changer, fill in the following order form
and print it out (if you fill out the parts that you can in a text editor
before you you print it, that makes it easier to read). Send the order form
with a check or money order for the proper amount of United States dollars
(see the following order form for price details), to the following address:
