Unique Identifier : AIDSLINE ASHM5/94348956 AB OBJECTIVE: To review the clinical and laboratory features, management and outcome of M.tuberculosis in 18 HIV seropositive patients (2.5% of the total AIDS patients). METHODS: A retrospective review of case records and laboratory results of 16 male and 2 female patients attending Fairfield Hospital between January 1985 and June 1993. The median age at presentation was 35 years (25-61 years) and 6 patients (33%) had no identifiable risk factors for TB. In 13/18 patients (72%), TB was the AIDS defining illness. There was less delay in the diagnosis of pulmonary TB than extrapulmonary TB (see table). 4/18 were treated presumptively for MAC infection prior to isolation of M.Tb. All isolates were fully sensitive to standard TB drugs and treatment was well tolerated. 9/18 patients have died and the median survival in the 8/9 patients diagnosed antemortem was 949 days (R173-1,724 days). CONCLUSION: TB occurs in advanced HIV disease, the diagnosis may be delayed, treatment is well tolerated and median survival is greater than 2.5 years. TABULAR DATA, SEE ABSTRACT VOLUME.
