Unique Identifier : AIDSLINE ASHM5/94348952 AB IgM antibodies against complex glycolipids, in particular GM1, GD1b and GQ1b gangliosides have been detected in the plasma of patients with acute and chronic inflammatory demyelinating polyneuropathy, multi-focal motor neuropathy and motor neurone disease. Inflammatory demyelinating neuropathy is one of several neuropathies that occur in patients with HIV infection. To determine the frequency of IgM antibodies against peripheral myelin glycolipids in HIV infection, 118 plasma samples were screened by enzyme-linked immunoassay (ELISA) and further analysed by Western blotting, indirect immunofluorescence and thin layer chromatography overlay. Thirty seven percent of samples showed IgM reactivity with purified myelin by ELISA, compared with only an occasional sample among 200 healthy blood donors. Reactivity could be abolished by prior incubation with purified myelin. No reactivity with myelin proteins was detected in any of the ELISA-positive samples. Reactivity with galactocerebroside was detected in 16 of 21 ELISA-positive samples and a further 7 samples showed reactivity with GD1b and GM1 gangliosides. These data suggest that immune damage to peripheral myelin in HIV infection may be mediated, in part, by IgM antibodies against glycolipids.
