Unique Identifier : AIDSLINE ICA10/94371603 AB OBJECTIVE: To determine effectiveness of the rolling down four layers training programme in improving the physicians skills in diagnosing and management of AIDS patients and reporting AIDS cases. METHODS: The peripheral physicians at the level of district and primary health centres have been trained in 1993-1994 on three major subjects: case diagnosis and management, counselling and reporting of AIDS cases. The advance of the skills of some 1500 physicians is being assessed to determine the effect of the training on diagnosis, recording and reporting. RESULTS: As a result of the training, the physicians first of all advanced their skills in diagnosis and clinical management of AIDS cases. The concept of counselling as a strategy to support sick people was realized by larger part of the trainees. The generally spread attitude of personal protection at the workplace was transformed into understanding of the importance of universal precautions. Physicians realized the importance of proper recording and reporting cases for the planning and advocacy purposes. DISCUSSION AND CONCLUSIONS: The centrally coordinated project was undertaken at a time when the administration of the States and Union territories, in this country with the federal administrative set up, just started developing their administrative and training capacity. This had advanced the training of the peripheral medical staff by one to two years. The central coordination also allowed to provide for interaction of the high level facilitators trained abroad with numerous peripheral staff in the whole country within a short period of six months.
