Unique Identifier : AIDSLINE ICA10/94371543 AB OBJECTIVES: Cost-benefit analysis of enteral Nutrition support (EN) compared with Total Parenteral nutrition (TPN) in patients with wasting syndrome in advanced AIDS with a decrease of more than 20% of body weight loss and infectious or tumoral complications. We do evaluate correlation between better nutrition and count of CD4 cells and Karnofsky index. METHODS: 100 patients were studied, all with a reduction of more than 20% of body weight for a period from two weeks to 3 months until improvement of undernutrition. CD4, Karnofsky index weight modifications cost of nutritional support and metabolic or infectious complications related to the Nutritional backup and mortality rate were evaluated. Group I of 40 patients that received TPN in the patients who presented intollerance to oral way due to esophagitis caused by candida or CMV, hypersecretory diarrhea with excreta between 2 to 7 liters by day, surgery, or incohercible emesis. 20 patients received TPN with dextrose 50%, ramificated chain aminoacids, and medium chain tryglicerides (MTC); Other 20 received similar composition with long chain triglycerides (LTC). Group II was formed by 60 patients and received enteral nutrition (EN) by oral way or nasoyeyunal tube. 25 patients received polymeric formula without fiber, 5 patients elemental formula, and 20 patients received IMMUNONUTRITION with MTC, A.A. enriched with arginine and gluthamine. RESULTS: In group I the average duration of TPN was 15.6 days (from 8 to 90), the increse of weight in one month was 8 kg (from 3 to 25 kg), Karnofsky index raised 20 points, CD4 increase in un medium of 25 cells (from 0 to 88), the cost of therapy was $3000.00 dollars, and 7 patients passed away, 6 with liver disfunction and one by pancreatitis, and 2 catheter infection, hyperglicemia in 8 patients. Group II had an average duration of 120 days (from20 to180 cost $2000.00DLLS.. weight increase 4 kg, I.I. in 15 points and CD4 increase only w. IMMUNONUT.
