Unique Identifier : AIDSLINE ICA10/94370426 AB AIMS: To determine the impact of the expanded 1993 European AIDS case definition on the distribution of adult AIDS cases in Europe. METHODS: The 1993 European AIDS surveillance case definition, implemented in the European region in 1993, comprises three new indicator diseases (pulmonary tuberculosis, recurrent pneumonia and invasive cervical cancer). Its impact on the surveillance of AIDS (defined as the total number of adult AIDS cases fitting solely the new case definition divided by the total number of AIDS cases diagnosed since the date of implementation in each country) was assessed using data from the European Non Aggregate AIDS Data Set (ENAADS) updated December 1993. The impact was also calculated by country and transmission category. RESULTS: A total of 471 AIDS cases fitting solely the new case definition were reported in 1993 by 27 of the 45 countries of the WHO European Region (impact: 471/9891 = 4.7%). Impact varied by country from 1.7% in Italy to 10.7% in Portugal and was high in some countries with small numbers of AIDS cases. Detailed break-down of 421 cases was available for 14 countries. In these countries 30% of the cases fitting solely the new definition were female, 65% presented with pulmonary tuberculosis, 28% with recurrent pneumonia and 7.4% with invasive cervical cancer. The impact by transmission category was: 7.6% for the heterosexual contact group, 5.7% for the injecting drug users (IDU), and 2.6% for the homo/bisexual group. CONCLUSION: At present the overall impact of the expanded 1993 AIDS case definition on surveillance is small (4.7%), although it remains to be assessed over a longer time period. Nevertheless the impact is more substantial in some countries and some transmission categories (heterosexual contact, IDU) and its implementation has enabled to better encompass the most severe manifestations of HIV infection.
