Unique Identifier : AIDSLINE ICA10/94369814 AB OBJECTIVE: To determine if saliva could serve as a non-invasive alternative to serum for testing of HIV antibodies in the on-going sentinel surveillance program in Thailand. METHODS: Serum and saliva specimens were gathered anonymously from 1,955 persons in four of 73 sentinel sites of the national surveillance program in Thailand. Saliva was gathered with the Omni-Sal collection device (SDS) and analyzed with the GACELISA (Murex). Serum served as the gold standard and was analyzed with an ELISA test for screening and the WB for confirmation. Linkage between serum and saliva was blind to the laboratory. Also analyzed blind was a set of HIV+ and HIV- quality assurance samples for both serum and saliva. RESULTS: The sensitivity of GACELISA with saliva was 98.0% in the field (298 HIV+), 100% after correction of errors (300 HIV+), and 100% among the quality assurance samples (95 HIV+). The specificity of GACELISA was 99.4% in the field (1,653 HIV-), 99.6% after correction of errors (1,654 HIV-), and 100% among the quality assurance samples (96 HIV-). CONCLUSION: Our findings in the largest study of its kind ever done, demonstrate that saliva is comparable to serum for assessment of HIV antibodies in a surveillance program. The next step is to develop a simple, inexpensive device for anonymous home screening.
