Unique Identifier : AIDSLINE ASHM6/95291892 AB This paper will explore the Clinical Nurse Consultant's role in implementing a community based model to clients living with HIV/AIDS. This program has been running for 2 years and recently incorporating care of persons living with haemophilia. The main aim of the position was: 1) To improve the quality and quantity of home based care for people living with HIV/AIDS and/or haemophilia. Objectives: 1) To provide a focus for client controlled case management within the region. 2) To work in close consultation within the community in the development and management of client services. 3) To advocate on behalf of the client his/her desired wishes/needs. 4) Deliver home based therapies to clients living with HIV/AIDS and/or haemophilia. 5) To provide bereavement follow-up to partners and family of HIV/AIDS for a twelve month duration. 6) To purchase services and goods from the brokerage fund on behalf of client's living with HIV/AIDS to prevent inappropriate institutional care. 7) To plan and co-ordinate HIV/AIDS education programs for health care providers. An evaluation of the position after twelve months duration will be explored. Within this evaluation the cost factor of home based care versus institutional care will be explored. Whilst numbers of clients in Tasmania have been relatively low the quality and quantity of care has been optimum. With HIV/AIDS services now being integrated into the Sexual Health model of service delivery a projected plan will be proposed for the future direction of HIV/AIDS and haemophilia care in Tasmania.
