Unique Identifier : AIDSLINE ASHM6/95291840 AB INTRODUCTION: Unexplained loss of lean body mass in HIV disease is common, particularly in the late stage, and contributes to reduced quality of life and survival time. This trend might be stabilised or reversed for a time with anabolic steroids and resitive exercise. METHOD: Patients who had lost 5-15% of their usual body weight were nutritionally counselled. Those who were not able to gain weight or who were unable to maintain any weight gained for 4 weeks were enrolled into this pilot study combining resistive exercise with nandrolone decanoate 100mg IMI every 2 weeks, for 16 weeks. Weight, lean body mass, quality of life, food intake and laboratory parameters were measured before and during the study. This abstract is a preliminary report on 11 subjects who completed the first 8 weeks of the study. Final data on subjects will also be presented. RESULTS: Overall there was a nett increase of 1.8kg +/- 1.9kg over the 8 week period (range 1.6kg to 3.8kg). This increase was predominantly lean body mass with mid arm muscle areas (a measure of somatic protein stores) increasing by 351 +/- 473 mm2 with no nett change in arm fat area. There was a nett increase in quality of life, as measured by the MOS SF30 questionnaire, with the largest improvements in the scales measuring vigorous activity/fatigue, positive affect and health distress. There were no significant changes to CD4+ cells, predominantly because the majority of the sample was already severely immunosupressed (average CD4+ cells 130). There was however, a trend towards increasing CD4+ numbers in subjects with initial numbers > 350.
