Most recent fixes:

Improved compatibility under Windows 95. 
  This was accomplished in LANCHECK.EXE V4.12.14.

Problem Report Number: T08434
  LANCHECK help states that NAME (Node Name) is case sensitive.   
  This is fixed in LANCHECK.HLP Revision: 1.10.

Problem Report Number: T06371
  Machine names longer than 10 characters will show incorrect Node Number.
  Node Number also does not display correctly from a L4W95 node that is  
  running LCHECK95.EXE.  
  This is fixed in LANCHECK.EXE V4.12.10.

Run LCH400.EXE in a temporary directory to extract the files.  Be 
sure to read this LCH400.TXT file carefully.  

Copy the updated files from your temporary directory to your \LANTASTI 
(or other directory) from which you will normally run LANCHECK.
