PCBPage registration is $15.00.  Registration entitles you to voice
support, and removes the "[UNREGISTERED COPY]" and "PCBPage x.x"
message, for a truly seamless interface.

You may  register PCBPage with Check or Money Order, Credit Card
payment is not currently available.

In  order  for you to receive a unique registration number,  you  must
provide the  name  of your system  EXACTLY  as it  is  entered in  the
PCBPAGE.CNF file, line #2.  Enter the name in the space below.  Please
circle any extended  ASCII  characters, and note their ASCII  Code for
me.  The  BEST  thing to do is load this form into a text  editor  and
enter the name of  your  system  exactly as it appears in  ALL of your
PCBPage configuration (.CFG) files.
