This is a Registration Form for single-computer users of BANK BOOK
for Windows. Please take your time and fill in as much information as possible
so that we can serve you better. Thank you very much.

If you want to use Bank Book on a network you have to sign a 
site-license agreement with us (it will be mailed to you after we receive your 
order). The price of a site-license can be calculated as the following:
Total = (Number of users) * $10.
For example, a 10-user license will cost $100 (10*$10).
The site-license price cannot be greater than $500, so if your network has
more than 50 users you will still pay only $500.

If you decide to register for BANK BOOK for Windows (see details
below) you can order any number of our Windows Collection programs
at discounted prices:
- regular prices $15 => reduced to $5;
- regular prices $20 and $25 => reduced to $10.

Just GO SWREG (ID# 3964).

It is a very convenient method of registering. You do not have to send us 
your registration fee by mail. Instead, CompuServe will add the registration 
fee to your monthly CompuServe bill. If you choose to register through
