This is a Registration Form for single-computer users of MOVIE CATALOG
for Windows. Please take your time and fill in as much information as possible
so that we can serve you better. Thank you very much.

If you want to use Movie Catalog on a network you have to sign a site-license 
agreement with us (it will be mailed to you after we receive your order). The
price of a site-license can be calculated as the following: 
Total = (Number of users) * $10.
For example, a 10-user license will cost $100 (10*$10).
The site-license price cannot be greater than $500, so if your network has
more than 50 users you will still pay only $500.

below) you can order 3 additional programs for only $15.

REGISTERING THROUGH COMPUSERVE:

You can register Movie Catalog through CompuServe Shareware Database.
Just GO SWREG (ID# 3840).

It is a very convenient method of registering. You do not have to send us 
your registration fee by mail. Instead, CompuServe will add the registration 
fee to your monthly CompuServe bill. If you choose to register through 
CompuServe however, please send us a message using CompuServe mail. We would 
appreciate it if you could fill out this form and include it in your e-mail
